Please be aware that the 2023 form 1095-C was sent out on Feb. 8, 2024, either by inner office mail or USPS. You will receive the 1095-C if you work 30 hours per week or more, even if you do not participate in the district health insurance plan.

Please note you do not need the 1095-C to complete your taxes. Form 1095-C does not get filed with your tax return, but it is recommended to keep a copy of the form with your tax records for future reference.
