Pioneer Central School district believes that students will become contributing members of society. Utilizing the Transition Planning Program, special education students will receive the necessary support to achieve this goal. Transition Services means a coordinated set of activities for a student with a disability, designed within an outcome-oriented process, that promotes movement from school to post-school activities, including, but not limited to, post-secondary education, vocational training, integrated competitive employment (including supported employment), continuing and adult education, adult services, independent living, or community participation. The coordinated set of activities must be based on an individual student’s needs, taking into account the student’s preferences and interests. Transition services will be integrated into academic, vocational, and work experience programs on an individual basis. Infusion of transition outcomes and services throughout these areas will also occur. The essence of our transition planning process incorporates the school professionals and/or outside agencies working together with individual families and their child to determine their transition process and outcome.
